Responsibilities
- Provide direct nursing care and support to patients in their homes or community settings.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Monitor patient progress and make necessary adjustments to care as needed.
- Administer medications and treatments as prescribed by healthcare providers.
- Educate patients and families about managing health conditions and promoting wellness.
- Collaborate with multidisciplinary teams to coordinate patient care and address health concerns.
- Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations.
Requirements
- Valid nursing license and registration with the appropriate regulatory body.
- Previous experience as a community nurse or in a similar healthcare setting is preferred.
- Strong clinical assessment skills and the ability to develop care plans.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Ability to work independently and manage time effectively in a community setting.
- Demonstrated ability to adapt to changing patient needs and environments.
- Current certification in Basic Life Support (BLS) and a commitment to ongoing professional development.
